Some members like to share their commitment with a spouse, partner, friend, or family member. If you’d like to include a teammate under the same $100 quarterly commitment, you can add their name here, and they will receive the same correspondence and reminders as the primary member. This is for one $100 commitment. If your partner would also like to become a committed member, please have them complete a separate form.

I understand that I am making a commitment to Jackson 100 For Good to make a $100 donation at each of four quarterly meetings, for a total annual donation of $400.

These donations will be made directly to local 501(c)(3) charities in the Jackson area. I agree that even if I did not vote for the charity chosen by majority vote, I will fulfill my donation commitment.

If I am not able to attend a quarterly meeting, I agree to still be responsible for the donation and will mail my check to P.O. Box 121, Jackson, Ohio, the day after the meeting, or drop it off at one of the designated drop-off locations.

I understand that new members may nominate a charity and vote beginning with the second meeting after they become a member, and at all subsequent meetings, provided they have made their contribution to the previous quarter’s chosen Impact Award Charity. I understand that I must have paid my $100 donation to the charity selected in the prior quarter in order to nominate a charity or vote at the following meeting.

My commitment will automatically renew for successive one-year periods unless notice is given.

I also acknowledge that photographs and videos taken at events and meetings may include my image and may be used in promotional materials for Jackson 100 For Good.

Members may resign at any time.

I understand that this agreement is an honor pledge and not a legal contract.
